Waiver

Acknowledgement, Waiver and Release from Liability: 

I confirm that I have not been advised against participation in Recreational gymnastics by a health professional. In consideration for allowing me to become a member of Mullingar Gymnastics Club and participate in class and events, I agree to the following: 

1. To abide by all Mullingar Gymnastics Club policies and 

procedures as amended from time to time and I acknowledge that my membership may be revoked or suspended for violation of these rules. 

2. To waive, release and discharge from any or all claims, losses or liabilities for death, personal injury, partial or permanent disability, property damage, medical expenses, theft or damage of any kind, including economic losses, which may in the future arise out of or related to my participation in recreational 

gymnastics classes, for whatever reason, the following person and entities: Mullingar Gymnastics Club, Fitness4U employees, Mullingar Gymnastics Club volunteers and representatives and agents of any of the above. 

3. Not to pursue a claim of any kinds against any of the persons or entities mentioned above in paragraph 2 for any claims, losses, or liabilities that I have waived, released, or discharged herein 

4. My membership details/records being held on a computer database by Fitness4U/ Mullingar Gymnastics Club.
